Talent Release Agreement

By responding to this offer, I hereby confirm that my services, name and image, as well as any other persons appearing, were rendered on a strictly voluntary basis for which I (we) do not expect compensation, financial or otherwise, of any kind. I agree NOT to include any Personal Health Information (PHI), and have not posted this video to social media.

mymedicalimages.com and their partners are granted the non-exclusive right to use and edit the written, video/photographic, or sound recording services (whether it be voice and/or instrumental) which I provide. This material may be used in any or all of the following: print, web, radio, video, still reproduction, television commercials, infomercials, informational videos, social media, internet or any other form for any legitimate purpose.

I further agree to hold mymedicalimages.com LLC and its officers, agents, directors, shareholders, employees and affiliated third parties harmless from any liability for violation of proprietary rights, copyright or right of privacy arising out of reproduction, use or disposition of any copyrighted materials. I acknowledge that I have read and understand all of the terms and conditions set forth herein.
